That is the only reason you would be capped at 60fps unless you have altered fps_max or used an external fps limiter

That right there is the biggest problem you still have... Even when given advice you just pretty much ignore it and come up with excuses not to do anything about it. Any information about your setup at this point helps.

Deepblue, look in your Nvidia/AMD control panel and check whether it's set to enable vsync either in all games or in TF2.

It's not enabled.
Both global and the game itself.

I heard about it a while ago and made sure it was off in every game I started playing ever since.

This is probably wrong, I tried setting them in IMG tags but they didn't show in the preview.
I don't upload that many images and imageshack started making you pay for an account so...

bring up console in TF2 and type fps_max.

make sure you have enabled developer console in advanced options. console is usually the key below escape key.

Also are you playing in windowed mode?

Which of those DPI settings are you using? 4 are configured, is the default 1000dpi?

And can you do a screengrab of windows control panel settings for the mouse. specifically the pointer options tab.

It is hard to tell if your mouse settings alter the main windows settings or not. If they do then you should have the mouse point speed bang in the centre.

So what should happen?
Is the code that pops up below the limit or something?

Because I'm getting 'fps_max 300'
Using just 'fps_max' on it's own tells me I can't set the max when connected to a server (whether I am or not) and using 'fps_max 300' sets it to 300 (I even went onto the a server to make sure it did cap at 300.

Not sure what will happen when there's a full server (or even when there are only a few people on)

I'm honestly not sure.
I don't try to mess with things I don't know anything about.

I change the speed and accel because I know what they do.

Since that's the only option you're concerned about.
It is in the centre.
Unless there's something else.
The point speed was the only one you mentioned.

Can you untick DPI2, DPI3 and DPI4?

You can connect to any server and type fps_max to see what the cap is currently set at.

If you raise it to 300 and net_graph 1 shows > 60 fps then you know that vsync is disabled.

300 is the normal setting, if it is any lower then you may have used a config or something at some point.
