I'd say Hard Rain in campaign and Dark Carnival for Versus play. Hard Rain has some amazing weather effects and I really like the concept of returning to previously visited areas. However, it loses quite a lot of its charm in VS, as many of its quirks are toned down (water doesn't slow you down as much, there are less witches, etc).
Swamp Fever has quite a few bugs in Vs, true. Other than that, I really like it, although the finale is arguably the worst one of the bunch, simply because it isn't all that special.

Over all it has to be Dark Carnival. Best setting/story for a campaign, and it has obviously had the most development time spent on it of the 5. Still bits of it could be better though. The tunnel of love is monotonous and the finale could've used some extra side geometry to break it up a little imo, it's too arena/coliseum like.

Dark Carnival is the best co-op map, as far as I have seen. It's also the hardest Expert campaign out of the whole bunch.
The finale is absolutely terrible in Versus, however. Huge open arena with the tanks running around in plain sight, lack of spawn points near survivors if they camp in corners, etc.
Dead Center has a really strong first level in Versus - plenty of really brutal deathtraps the Infected team can make use of. The Mall level, however, is absolutely terrible, and the finale plays out just like a game of Scavenge on this map - good teams can escape before the first tank arrives, what the hell.
Hard Rain is really short and consists mostly out of huge open spaces. Althought the survivors aren't supposed to be able to see in fog/bushes/cornfields, you still can't spawn there. Brutal lack of SI spawn points, and there's absolutely nothing you can do to stop a team of rushers on this map. Terrible Versus experience.
Swamp Fever also has a huge issue with spawn blocking. Massive open areas with no chokepoints do not make for fun Infected gameplay, Valve!
Parish has an awesome finale, smart Versus-oriented map design and is a blast in co-op as well. By far the best all-round campaign.
